JOURNEYCARE® is a nonprofit organization that provides healthcare and hospice services to individuals, families and communities in Chicagoland and Southern Wisconsin. Founded in 1978 with the intention of providing the highest quality of care, JOURNEYCARE® has grown to serve more than 18,000 patients and families each year.
JOURNEYCARE® offers a range of services from routine checkups to end-of-life care. These services include home healthcare, hospice and palliative care, respite care, day programs, pharmacy services, chaplain services, bereavement counseling, advanced care planning and more. JOURNEYCARE®s team of healthcare professionals consists of physicians, nurse practitioners, registered nurses, certified nursing assistants, social workers, chaplains and certified counselors.

JOURNEYCARE® advocates for each patient’s right to make informed decisions about life-sustaining treatments and end-of-life care. JOURNEYCARE® works in collaboration with multiple organizations across the region to ensure everyone in their community has access to the highest quality of care. In addition, JOURNEYCARE® recognizes the importance of volunteer service and welcomes individuals and groups to provide companionship and support to those in need.
