AGRACE HospiceCare is a community-based, nonprofit, nonprofit health organization dedicated to providing ten-county communities in south-central Wisconsin with the highest quality end-of-life and supportive care services available. Founded in 1978, AGRACE has since been providing compassionate care and emotional and spiritual support to individuals, families and caregivers facing the challenges of a life-limiting serious illness.

AGRACE HospiceCare strives to eliminate suffering to ensure dignity and quality of life during the time a person faces a life-limiting condition, regardless of the resources available. This is done through services such as symptom management; pain and symptom management; emotional and spiritual support for patients, families and caregivers; and short-term homemaker and home health services.

AGRACE HospiceCare serves individuals and families from all walks of life, from all beliefs and cultures. Its services can be provided in any setting of comfortable choice, including the home, personal care facilities and hospitals. All of AGRACE’s care is tailored to the unique needs of its patients and families, offering medical, social, emotional and spiritual support. The services provided by AGRACE also include education about end-of-life choices and counseling for family and caregivers. At the core of AGRACE is their commitment to creating a safe and comfortable environment that allows individuals to accept and plan for their life-ending illness.

My family and I could not have asked for better care than we received from the team at AGRACE HospiceCare during the final days of my mom’s life. From nurses to technicians to volunteers, everyone was attentive, kind, and generous with their time and efforts in providing a safe and comfortable environment that allowed us all to make sensitive memories with our mother before her passing. Time spent with AGRACE caregivers felt like time spent surrounded by dedicated friends reminded us that we were not alone but when it came down it palliative care was second to none! Communication was exceptional throughout; prompt responses eased any questions or concerns we had as well as made sure our loved one felt supported right until the end
