CHAPTERS HEALTH® SYSTEM is a not-for-profit organization that provides comprehensive hospice and palliative healthcare services to those in Southwest and Central Florida– from Hillsborough, Pasco, and Pinellas counties of the Tampa Bay area to Polk, Highlands, Hardee, and Manatee counties in Central Florida. Founded in 1984 with the goal of delivering care and support to those with life-limiting illnesses, CHAPTERS HEALTH® SYSTEM has grown to become a reputable source of hospice care and support programs. CHAPTERS HEALTH® SYSTEM is an approved Medicare and Medicaid provider, as well as licensed by the Florida Agency of Health Care Administration (AHCA).

CHAPTERS HEALTH® SYSTEM provides an extensive array of services to its patients, including physical and emotional comfort, spiritual support, pain and symptom management, counseling, respite care, durable medical equipment, and more. Through its compassionate staff of nurses, hospice aides, social workers, counselors, therapists, and volunteers, patients can receive individualized, holistic care designed to provide emotional, physical, and spiritual support throughout their end-of-life journey.

CHAPTERS HEALTH® SYSTEM also offers bereavement support programs for family, friends, and caregivers of those who have passed away. Support is offered through individual and group sessions, online resources, a bereavement newsletter, memory books, and more.

Chapters Health® System provides excellent palliative care and hospice services to my patients and their families. The entire staff is welcoming, caring and patient-focused always providing quality of life above all else. Communication with the staff was good during this time for them as well as for me in order to ensure that our goals align throughout the process. The attention given by the team members directly involved with my patient’s care was thoughtful, understanding yet professional – shining a light into an otherwise dark situation. Excellent job!

My experience with Chapters Health® System has been nothing short of amazing from start to finish – especially around end of life wishes for those I love most deeply so it takes great expertise, comfortability and grace which are certainly reflected in these very kind individuals who truly understand what matters driving home moments before passing on surrounded by ones you’ll hold dear forever! Staff provided exceptional communication not just within family but gave reassurance/updates to treating docs like myself as plans or concerns shift along journey so we’re all on same page working together

My uncle recently passed away while under hospice care through Chapters Health® System and I must say everyone (doctors, nurses & caregivers) were incredibly compassionate & comforting- taking extra time after hours refusing additional pay because they knew how difficult it can be seeing a family member pass away like that…They really went out of their way making us feel safe allowing us more quality time at his bedside…. it was incredible watching such selfless people put others needs first . To top off warm atmosphere he received superior medical attention giving him a proper goodbye..Thumbs up !
