JOURNEYCARE® is a nonprofit organization that provides healthcare and hospice services to individuals, families and communities in Chicagoland and Southern Wisconsin. Founded in 1978 with the intention of providing the highest quality of care, JOURNEYCARE® has grown to serve more than 18,000 patients and families each year.

JOURNEYCARE® offers a range of services from routine checkups to end-of-life care. These services include home healthcare, hospice and palliative care, respite care, day programs, pharmacy services, chaplain services, bereavement counseling, advanced care planning and more. JOURNEYCARE®s team of healthcare professionals consists of physicians, nurse practitioners, registered nurses, certified nursing assistants, social workers, chaplains and certified counselors.

My grandmother has been receiving end-of-life care from JOURNEYCARE®, and our entire family could not be happier with the experience they’ve had so far! The staff quickly put together an appropriate plan that was designed around her specific needs, taking into consideration both physical & emotional concerns as well as making sure she got the best possible quality of life along the journeyin her last days here on earth! JourneyCare provides outstanding services to those going through end-of-life transitions or dealing with chronic illness management at home or in facilities – period! Their teams go above & beyond when implementing individualized individualized plans – making sure each patient gets accurate diagnosis & treatments plus specialized therapies if needed; warm companionship during visits; coordination among providers; open communication between clinicians & concerned family members; timely access to reserves AND social workers..no question there is no better hospice out there right now!!-
